Elderlynk House Calls

Title: "Elderlynk: House Calls Program" Brief Description: Interdisciplinary teams visit their patient four times over the course of two years. Each visit is part of a comprehensive geriatric assessment to address and monitor patient health and personal concerns. Objectives: To provide basic health care to the elderly who receive inadequate amounts (primary), and to help deal with the shortage of health care providers to the growing elderly population (secondary). Degree to which objectives were met: Although the program is not complete, the primary objective has been met. Implications for Health Education: If proven highly successful, other organizations can adopt this program in an effort to reduce the number of untreated elderly. Conclusions: This program appears to be meeting its objectives and proving successful. Relatively new in practice, it has the makings to become a widespread and well-adopted program. This research was conducted at Truman State University.
