Determining Markers that Correlate to a Reduced Chance of Readmission in Chronic Disease Patients

Currently, 1/4 of heart failure patients are readmitted within 28 days of being released. The purpose of this study is to determine if improvements in certain tests correlate to a reduced chance of readmission in all chronic disease patients, not just heart failure. Some of these tests include the 6-Minute Walk Test and the Incremental Shuttle Walk Test. Due to the small number of participants, this will be an ongoing study in the future.
