Patient Navigators for a Diabetes Self-Management Program: Tracking Progress and Wellness of Diabetes Patients

Started in November 2007, CareLink Better Self Management of Diabetes is a program in which people with diabetes attend three free two hour informational classes about the care and management of their illness. The participants set goals at the last class and are contacted monthly via telephone on their progress and general health. After being asked a series of 15 health and wellness questions, 58% of the 173 patients reported they have checked their blood sugar the number of times recommended by a physician during the seven days prior to the phone call and 89% have taken their diabetes medication as directed during the seven days prior to the phone call. Between July 1, 2008 and December 31, 2008, members of Eta Sigma Gamma have made 129 contacts through the program.
